Please print off and complete the following forms as needed and turn them into the Guest Service Desk or your department leader for approval.

Volunteer Application Form If you are interested in serving in area of the church.

Event Request Form  Please complete this form if you are interested in cooridnating an event for Lapeer Community Church.

Facilities Use Application and Aggrement Forms Please complete BOTH of these forms if you would like to use Lapeer Community Church property for a private rental or are an outside organization's event.

Wedding Planner Form  Please complete this form if you would like either of our pastor's to conduct a wedding ceremony for you.  This form should be complete for weddings that will occur on or off site.  If you would like to use LCC facilities for your wedding, you must also complete the Facilities Use Application and Aggrement Forms.

Business Network Form Please complete this form if you would like to be included in our newsletter's business network section and included on our website.
